You have been tired for months. Your stomach is unpredictable. You have lost a few pounds without trying, and your most recent blood panel flagged a mild anemia your doctor attributed to diet. You have been told it might be irritable bowel syndrome, or stress, or possibly a thyroid issue that needs monitoring. The explanations feel plausible but incomplete—because nothing you try seems to move the needle.

For a subset of Americans in exactly this position, the actual explanation sits in a diagnostic category that rarely appears on the differential: a parasitic infection.

The Diagnostic Gap in American Medicine

The United States healthcare system is extraordinarily well-equipped to identify and treat acute conditions. Parasitic infections, however, occupy an uncomfortable middle ground. They are chronic, often indolent, and their symptoms overlap extensively with conditions that are far more familiar to American clinicians—irritable bowel syndrome (IBS), iron-deficiency anemia, fibromyalgia, chronic fatigue syndrome, and generalized anxiety disorder among them.

Medical education in the US dedicates relatively limited curriculum time to helminthic and protozoal infections compared to training programs in countries where these diseases carry higher endemic burdens. The practical result is that primary care physicians, internists, and even gastroenterologists may not reflexively order parasite-specific diagnostics when a patient presents with fatigue, GI distress, or unexplained weight loss—particularly if that patient has not recently traveled internationally.

This assumption—that parasites are primarily a travel medicine concern—is one of the field's more consequential blind spots. Several parasitic infections are endemic within the United States or are introduced through domestic food supply chains, immigration patterns, and everyday environmental exposures that have nothing to do with foreign travel.

How Parasites Produce Symptoms That Mimic Other Conditions

Understanding the mechanism helps explain the diagnostic confusion. Intestinal helminths and protozoa interact with the human body in ways that produce systemic effects far beyond the gut itself.

Iron-deficiency anemia is among the most clinically significant consequences of hookworm infection. Adult hookworms attach to the intestinal wall and feed on blood—each worm extracting a small but continuous volume daily. In moderate to heavy infections, cumulative blood loss is sufficient to produce measurable anemia, with fatigue, pallor, and reduced exercise tolerance as downstream effects. A patient presenting with these findings may receive iron supplementation and dietary counseling without anyone considering whether worms are actively depleting what the supplements replace.

IBS-like symptoms—bloating, alternating constipation and diarrhea, cramping, and excess gas—are characteristic of Giardia lamblia infection, whipworm (Trichuris trichiura), and in some cases, Strongyloides stercoralis. These symptoms are clinically indistinguishable from functional gastrointestinal disorders without targeted testing. Patients who meet Rome IV diagnostic criteria for IBS but fail to respond to standard management may warrant a more thorough infectious workup.

Chronic fatigue without a clear structural explanation can reflect the metabolic burden of a sustained parasitic infection. The immune system's ongoing response to helminth presence is energetically costly. Cytokine release, inflammatory signaling, and nutritional competition from the parasites themselves combine to produce a persistent low-grade exhaustion that standard fatigue screening rarely attributes to infectious causes.

Eosinophilia—an elevated count of a specific white blood cell type—is a laboratory finding that should immediately raise clinical suspicion for helminth infection. Unfortunately, eosinophilia is sometimes noted incidentally and not pursued aggressively, particularly when the patient's travel history appears unremarkable.

Red Flag Symptom Combinations Worth Raising With Your Doctor

No single symptom reliably points to a parasitic cause. It is the combination and context that matter. The following clusters warrant explicit conversation with a healthcare provider about parasite screening:

When raising these concerns with a clinician, requesting specific parasite-focused diagnostics is appropriate. Standard stool culture does not reliably detect helminths. Ova and parasite (O&P) examination of stool—ideally from multiple samples collected on different days—is the conventional approach. Serology and PCR-based testing are available for specific organisms and may be recommended in certain clinical contexts.

Why Parasitic Infections Are Underdiagnosed in the US

Several structural factors contribute to the diagnostic gap beyond physician training alone.

Insurance reimbursement structures in American healthcare incentivize rapid throughput in primary care visits. A fifteen-minute appointment is rarely sufficient to take the kind of thorough environmental and dietary history that might surface parasitic exposure risk. Patients may not volunteer information about dietary habits, gardening practices, pet contact, or consumption of raw or undercooked foods unless directly asked.

Additionally, mild to moderate parasitic infections frequently do not produce dramatic acute symptoms. A patient with a moderate hookworm burden may feel persistently unwell rather than acutely ill—a presentation that generates less clinical urgency and more diagnostic drift.

Finally, there is a cultural assumption embedded in American medicine that parasites are a developing-world problem. This assumption is factually inaccurate. Toxocariasis, strongyloidiasis, and trichinellosis all occur domestically. Pinworm infection is among the most prevalent parasitic conditions in the country, affecting millions of children and adults annually. Giardia is consistently among the most reported parasitic causes of diarrheal illness in the US according to CDC surveillance data.

OTC Anthelmintics: An Accessible Option Once Parasites Are Identified

When a parasitic infection is confirmed—or when clinical suspicion is sufficiently high and a healthcare provider recommends empiric treatment—over-the-counter anthelmintic medications offer a practical, evidence-based treatment pathway.

Albendazole is a broad-spectrum anthelmintic effective against hookworm, roundworm, whipworm, pinworm, and several other helminth species. Its broad efficacy profile makes it particularly useful in cases where the specific species has not been definitively identified but helminth infection is strongly suspected. Albendazole works by disrupting the parasite's ability to absorb glucose, leading to energy depletion and death of the organism.

Mebendazole is similarly effective against the most common intestinal helminths and has a well-established safety record. It is particularly well-suited for pinworm treatment and household decontamination protocols, where simultaneous treatment of all household members is typically recommended.

Both medications are generally well tolerated in healthy adults at standard doses. Individuals with liver conditions, those who are pregnant, or those managing complex comorbidities should consult a healthcare provider before initiating anthelmintic therapy. It is equally important to recognize that protozoal infections—including Giardia and Cryptosporidium—require different medications entirely and will not respond to standard anthelmintic agents.

Advocating for Yourself in the Exam Room

If you have experienced a constellation of unexplained symptoms for an extended period and feel your care has reached a diagnostic plateau, raising the possibility of parasitic infection directly with your clinician is a reasonable and legitimate step. Requesting O&P stool testing, a complete blood count with differential (to evaluate eosinophil levels), and a thorough review of your dietary and environmental history is appropriate patient advocacy—not alarmism.

For symptoms that align with common helminth infections and for which OTC treatment is suitable, albendazole and mebendazole represent accessible, affordable options that do not require navigating lengthy specialist referral processes. Understanding what these medications treat, how they work, and when they are appropriate is precisely the kind of informed health literacy that leads to better outcomes.

Chronic symptoms deserve thorough investigation. Parasitic infection may not be the answer in every case—but it should not be the last possibility considered simply because it is unfamiliar.
