Dexamethasone, a corticosteroid, and cimetidine, an older acid-reducing medication, have both been shown in clinical studies to increase plasma concentrations of albendazole sulfoxide. In the treatment of serious systemic parasitic infections like neurocysticercosis, physicians sometimes use dexamethasone intentionally alongside albendazole. While this combination is sometimes used intentionally in clinical settings, consumers should not combine antiparasitic medications without medical supervision.

HIV Medications and Other CYP Inducers/Inhibitors

Several antiretroviral drugs used in HIV treatment — particularly ritonavir and other protease inhibitors — are powerful inhibitors or inducers of cytochrome P450 pathways. Individuals managing HIV who also need anthelmintic treatment should always consult their HIV specialist or clinical pharmacist before taking albendazole, as interactions can affect both drug efficacy and safety.

Interactions With Common OTC Medications

Antacids and Acid-Reducing Agents

The absorption of albendazole is significantly enhanced when taken with a fatty meal — a fact that appears on the medication's prescribing information. However, the role of gastric pH is also relevant. Antacids and proton pump inhibitors (such as omeprazole or lansoprazole, sold as Prilosec and Prevacid) alter the stomach's acidic environment in ways that could theoretically influence drug solubility and absorption. While this interaction is not as well-characterized as the CYP-related ones, consumers taking regular acid suppressants should mention this to their pharmacist.

Laxatives and Anti-Diarrheal Medications

Because mebendazole's mechanism depends on local GI action, anything that significantly accelerates or slows intestinal transit may affect how well the drug works. Using stimulant laxatives or anti-diarrheal agents like loperamide (Imodium) around the time of mebendazole treatment is not recommended without pharmacist guidance.

St. John's Wort

St. John's Wort is one of the most potent herbal inducers of cytochrome P450 enzymes known. Regular use of this supplement can significantly reduce albendazole sulfoxide concentrations in the blood, potentially rendering treatment ineffective. If you take St. John's Wort for mood support, discuss timing and alternatives with a healthcare provider before starting albendazole.

High-Dose Grapefruit Consumption

Grapefruit and grapefruit juice inhibit CYP3A4, an enzyme involved in albendazole's metabolism. While the clinical significance for albendazole specifically is not as well-documented as it is for certain statins or calcium channel blockers, erring on the side of caution and avoiding large quantities of grapefruit during albendazole treatment is reasonable.

Iron and Zinc Supplements

Some research suggests that high-dose mineral supplementation may interfere with mebendazole absorption. If you are taking iron or zinc supplements — common in individuals treating parasite-associated nutritional deficiencies — spacing these out from mebendazole doses is advisable.
